Does it look like the sample medical screening form I have posted in the discussion area? That was from my cruise on the Roosevelt last year...there is a place for doctor comments if required....if you are in good health with no medical flags I don't see why your Dr. would have to fill it out....but things do change. Advice if you have not had a tetanus booster in while you bite the bullet and get a shot.....putting up with a sore arm for a week is worth it once you get on the carrier.

I filled my form out and scanned it and sent it back via e-mail to speed things up.

Thank you for the information. Th form for the abraham lincoln is very similar to the one you posted. I did not think a physician needed to fill the form out but my son kept saying i needed to see my physician. I am looking forward to my first cruise and will definitely read the blogs .
